Creation of the Mirabror Usmanov International Charitable Public Foundation
In Uzbekistan, the principle of «Reform not for the sake of reform, but for the benefit of people» is proclaimed and implemented in practice. To this end, special attention is being paid to the social protection of citizens and to the implementation of a coherent and targeted State policy to support the most vulnerable segments of society, orphans and children left without parental care, and persons with disabilities.

How to get help
The following documents are required in order to receive charitable assistance
Application to the MIRABROV USMANOV Charitable Foundation
Passport copy (child's birth certificate)
TIN copy
Original certificate from the Mahalla committee that the applicant is on the list of low-income families
Original act of inspection of the mahalla committee on the applicant's living and financial conditions. (with obligatory indication of fist name and last name of mahalla chairman, address and telephone number of mahalla)
Full information (full name, telephone number of the chairman of the mahalla committee and the district officer) address of the mahalla committee
Copies of documents confirming the applicant's disability
Extracts from medical records with indication of the medical institution
Documents confirming the diagnosis (tests, X-rays and other examinations)
Documents from clinics confirming the need for medical care (surgery, treatment, examination, etc.)
Photos of the patient and the applicant's living conditions.
Note: During the study of submitted documents, the MIRABROR USMANOV Foundation has the right to request additional documents and information from the Applicant.

Answer: The MIRABROR USMANOV International Charitable Public Foundation is involved in the social protection of vulnerable people, provides targeted assistance to the most needy people and assists them in obtaining specialized medical care, and supports orphans and persons with disabilities. The Foundation also helps to improve the rehabilitation of these people and to create the necessary conditions for their full participation in the social and public life of the country.
Answer : The following documents are mandatory to be submitted for charitable assistance:
- Application to the MIRABROV USMANOV Charitable Foundation
- Copy of patient’s passport
- TIN copy
- For underage patients, a copy of the birth certificate and copies of passports, as well as copies of the parents’ TIN
- Original certificate from the Mahalla committee. The certificate should specify:
that the applicant is on the list of low-income families or that he/she needs charitable assistance, information about the composition of the family members (of them, are employed), address of the makhalla committee, full name of the chairperson, his telephone number. The

Answer : The Foundation cannot provide medical advice. A child can only be diagnosed and get any other medical advice at a medical institution.
Answer: Yes, you can choose the medical organization of your choice to treat your child. We can also advise you on the choice of medical institutions with which our foundation cooperates.
